1)  Pets less than two (2) months (8 weeks) are not transportable across state lines without their mother per the way Operation Roger understands the USDA regulations. 

2)  Operation Roger's Policy: Puppies/Kittens 8-12 weeks coming directly from a shelter will not be transported except for the following: Must be observed for Parvo &/or Distemper in a Foster/Rescue/Separate containment at Shelter for two (2) weeks minimum. 

3)  Puppies should have had the second of two (2) puppy shots preferably two (2) weeks prior to transport.  Kittens, their respective feline shots. 

4)  Each pet is to have been wormed during this same time frame and be worm free. 

5)  As commercial truck drivers, we must comply with all federal and state laws regarding our profession and satisfy any inquiries which might be made by Motor Carrier Enforcement or other law enforcement agencies.  This is for the protection of our clients, our volunteer drivers, our organization, and above all, the pet entrusted to our care during transport.

For Operation Roger, as we are currently a small group of volunteer truck drivers with freight time schedules and destinations, a time frame of less than 4-6 weeks to move your pet is considered an emergency (it can be done if a driver is virtually sitting on top of or actually heading toward the pet); 2-3 or more months could happen if in a difficult area or without cooperation from the shipper. 

ALSO, There are just some states and some areas within states which are extremely difficult to service.  We will let you know what the obstacles are for the shipper.

1)                  Shipper understands they are to meet the Driver at the agreed upon location and at the appointed time so as notto delay the Driver in his/her regular schedule.  If the Shipper is not at the location and the Driver must leave in order to make his/her scheduled appointment time for delivering the freight, the pet will miss a ride.

2)                  Operation Roger will reinstate the pet to the active Pet Board and look for another driver.  We cannot guaranteehow long this will take.

2PT7-1 MEDICAL FINANCIAL EMERGENCY AGREEMENT 

1) The individual listed below is named as the person responsible for paying any and all veterinary services required by the pet named in this application while on a truck, with a shuttle driver entrusted with this pet, in a layover home, or with Operation Roger Truckers Pet Transport (herein after known as Operation Roger) drivers.
2) This individual, whether they are the Applicant/Shipper/Receiver/Another individual involved in desiring Operation Roger to transport named pet does hereby release, discharge, and hold harmless Operation Roger, any person, firm or corporation charged or chargeable with liability together with their heirs, administrators, executors, successors and assigns from any and all claims, damages, costs, expenses, loss of services, actions or causes of action, whether known or unknown, for any illness, injury, or loss of life to the pet, or injury to other pets, Operation Roger drivers, or other people occurring during the transport of the pet, belonging to the volunteer transport family due to any action or occurrence arising in connection with the transport of a pet for Operation Roger and its sending/receiving/rescues/shelters/individuals. 
3) Applicant/Shipper/Receiver/Individual named below hereby authorizes any law enforcement, humane lawenforcement official, or individual from a rescue or shelter to seek treatment for the pet listed on this Application from a licensed veterinarian in the event of an emergency.
4)  Any pet who isn't bathed prior to transport, is found to stink, or is dirty or has fleas/ticks after it is enroute on thetruck may be taken to a groomer at the option of the Driver/Layover Home/Shuttle Driver/Operation Roger at double the cost.  This may also include a mandatory securement of a motel room in order to bathe said pet at the same double cost.  Operation Roger has the right to withhold delivery of pet until such remuneration is received in the officeon behalf of the driver incuring the expense or paid directly to the Operation Roger driver incurring such cost.  Any misrepresentation pertaining to the pet being transported resulting in any of the afore mentioned events occurring, will be the financial responsibility of the person named below.

2PT7-5 FYI PARVO INFORMATION 

January 2014:  Researchers have put statistics together on the Parvo Virus and recently discovered the immune systems of both the German Shepherd and Rottweiler breeds are extremely susceptible.
When these breeds do get Parvo, their cases are more severe.  The death rate, even with A-1 care, is nearly 100% for all pups under a year old even with all their shots.  There has been reports of dogs just a few days shy of a year getting and dying of the disease.
Parvo is everywhere.  We and our pets can walk where an infected animal has been and bring it into our homes. Once the pet licks its paws and ingests what is there, their immune system may or may not be strong enough to fight it.
The veterinarian recommended time frame for at least three (3) Parvo shots three (3) weeks apart.  It doesn't hurt the animal to have several of these shots during that first year.  It just gives them more protection.
The symptoms are not eating, not drinking, vomiting, bloody diarrhea, face looks depressed, eyes become dull, and high fever.
If you suspect your pet of any breed has Parvo, immediately get it to a veterinarian and on continuous IVs as their system will dehydrate very, very quickly.

Q1) What do I need to take my dog or cat to Canada or Mexico? 

A) Generally, these countries require proof of current rabies vaccination, i.e. a rabies certificate and a health certificate signed by your veterinarian. This health certificate does not have to be stamped/ endorsed by our office. Q2) What do I need to take my pet to another state within the United States? 
A) USDA does not set requirements for the movement of pets across state line. The requirements are actually set by each individual state. Therefore, you should contact the State Veterinarian of the state to which you are traveling. They will provide you with the requirements. Below are two helpful websites for interstate movement of pets.

A) USDA is not the lead agency in regulating the entry of cats and dogs into the United States. The state of destination and the Centers for Disease and Prevention (CDC) regulate these animal entries. Generally, the animal will need a valid rabies vaccination and a health certificate. We ask you to please contact the state veterinarian in your state of destination and review the information at the following CDC website for more specific information.  http://www.cdc.gov/ncidod/dq/animal.htm 

Q4) IRegs for Animal Exports, Last Modified: Feb 3, 2015 

A) International health certificates for the export of animals from the United States are completed by the accredited veterinarian who certifies herd and animal health status, conducts tests, and records test results for the individual animals being exported. Completed and signed international health certificates for the export of animals from the United States must be endorsed by a Veterinary Services area office in order to be valid. 

A) The United States has minimal requirements for animals to be exported to other countries. Your Area 

Veterinarian-in-Charge can provide you with the current regulations, tests, and inspections required. Approved ports of embarkation and shipping requirements can be found in 9 CFR 91. Each country may have other specific health requirements for entry of animals. These requirements are established by the importing country, not the United States. Other countries may also have their own certificate format for export. Since export requirements frequently change, obtain the current export requirements from the Veterinary Service office in your area before each shipment. Do not rely solely on information provided by brokers and exporters. Export certificates are official documents and they should be typewritten, accurate, and complete. 

A) If you have any questions or concerns regarding these regulations for exporting animals or animal products to a foreign country, you should contact the APHIS-VS Service Centers in the State from which the animals or products will be exported. 

IMPORT HEALTH REQUIREMENTS OF CANADA FOR DOGS FROM THE UNITED STATES 

The following categories of dogs only need proof of a rabies vaccination within the 3 years prior to export to 

Canada: (a) any dog greater than 8 months of age; 

(b) an assistance dog that is certified as a guide, hearing or service dog, if the person  accompanying the dog toCanada is the user of the dog; 

(c)  not more than two pet dogs accompanied by their owner to Canada; and 

(d) any dog exported on a temporary basis for competition in a show or trial, if at the time  of importation proof ofentry in a show or trial is provided. All dogs greater than 3 months of age, with the exception of assistance dogs certified as a guide, hearing or service dogs which accompany their user into Canada, must have proof of a rabies vaccination within 3 years prior to importation. This proof can be in the form of a rabies vaccination certificate or the vaccination can be documented on the health certificate, if a health certificate is required. 

Canadian Border:  These certificates do not need to be endorsed at the Area Office - the only signature required is that of the licensed or accredited veterinarian, as applicable. All puppies will be examined by an Agriculture Canada inspector at the first port of entry.

2PT10-1 ITEMS REQUIRED FOR TRANSPORT 

The cost of any items needed and not provided by the Shipper to the Driver and having to be purchased by the Driver for the safety and comfort of the pet, MUST be reimbursed: 

1)  Directly to the Driver UPON delivery and when presented with a receipt. 

2)  Directly to Operation Roger through PayPal for the full amount plus PayPal charges when we present receipts(via email) from a Driver who was not the one who delivered the pet.  We will then reimburse the Driver. 

3)  If Driver must take pet for grooming or obtain a motel room because it stinks/dirty/fleas/ticks and needs a bath,the charge to the responsible party may be double the actual cost to reimburse the Driver for his/her time and effort. This may be reduced to the actual amount upon presentation directly to the Groomer/Motel of your credit/debit card information over the phone. 

4)  ALL of items in the List below are Required for transport with few variations. 

5)  The following items are mandatory for all pets except the rattie community: 

Health Certificate, Shot Records, Collar AND Harness hooked together, Crate (30 pounds & under), food and water bowls, food 15-30 days, recent bath. Choose EACH item to be sent with the pet.  Any missing item could result in the pet missing a ride. Note the Required items.

2PT10-2 FINANCIAL COST FOR TRANSPORT ITEMS 

1)                  The financial cost for the items required for transport is usually paid by the Receiver, although it can be theshipper or another person can be designated.
2)                  Operation Roger Volunteer Drivers should not be excepted to pay for anything needed by the pet duringtransport.
3)                  The TYPED Name of the Responsible Person and Relationship to the Pet as shown below is accepted the sameas a written signature.

2PT11-1 APPLICANT RESPONSIBILITY
1)  As the individual (herein called the APPLICANT) submitting this Pet Transport Application with OperationRoger Truckers Pet Transport, it is YOUR responsibility to notify the SHIPPER and/or the RECEIVER/or BOTH as the case may be, of our transport requirements according to the List on this Application as well as the Requirements Page.
2)  As the APPLICANT, it is extremely important you provide the SHIPPER a list of required items so they willknow exactly what we need from them in order to pick up the pet.
3)  We have had Shippers not even know we are involved in the moving of this pet.
4)  The Driver could refuse to take the pet brought to them without food, bowls, proper securement devices, etc.Click on the Box below:

2PT11-2 VOLUNTEER DRIVERS 

1)    The APPLICANT understands Operation Roger Truckers Pet Transport is made up of volunteer drivers.  We areNot a trucking company.
2)    We do Not dispatch our Drivers on loads and thus must wait for an available driver to be in the right area, at theright time, and going the right direction for a transport to happen.
3)     Because of who we are and what we do for a living, we cannot guarantee a time frame for either pick up ordelivery or even if we can pick up your pet in the time frame the Applicant/Shipper/Receiver desire.
4)     Because of this, if the Applicant/Shipper/Receiver decides to Not wait for a Operation Roger Volunteer Driverand finds another form of transportation for the pet named herein, the donation is understood to be Non-Refundable.
  It is tax-deductible as we are a 501(c) Non-Profit Charity.  The Donation is NOT for the transport.  The transport itself is Free.
5)     Operation Roger will diligently keep the herein named pet before the Active Drivers for however long it takes,as long as we have cooperation from both the Shipper and the Receiver.
6)      If the Shipper and/or the Receiver does not adhere to our requirements, does not return calls/texts, refuses tomeet a Driver on the Driver's time schedule when given sufficient time to do so, or in some way keep an Operation Roger Volunteer Driver from picking up a pet for a maximum of three (3) times, the pet will still be on the Active Pet Board but could be put behind another pet whose Shipper can cooperate.
7)     We do NOT keep a list, per se, of the pets to be transported and move them up and down that list.  All pets areequal.  Which ones are picked up first depends upon if there is a Volunteer Driver in that area and able to take that particular type of pet.
8) Some drivers are allergic to cats.  Others can only take cats.  Each driver has the right to chose to take what petthey want in their truck.  After all, it IS their truck and they have the final say.
9)    If the pet is transported by other means, we request the Applicant/Shipper/Receiver notify us immediately so wecan work on other transports.  We feel it is not who transports, but that the pet arrives safe and sound.

2PT11-3 TRUCKING REQUIREMENTS
1)  Applicant/Shipper/Receiver MUST understand Operation Roger Truckers Pet Transport Volunteer Drivers aresubject to the requirements of our occupation. 
Due to the nature of the trucking business, the loads of freight we haul are time critical.
2)  Pets MUST be brought to or picked up from the driver at a truck stop, rest area, or agreed upon location the Driver can safely & legally get his/her truck into AND at a time when the Driver arrives, whether it is 1 p.m. or 1 a.m.
3)  Operation Roger CANNOT guarantee a time frame for pickup or delivery or even if we can get to the pet at allin the time frame the parties involved want.  We do pledge to try.
4)  We have had pets picked up within two hours of reaching our Active Pet Board and others have been there forseveral months.  There are numerous factors involved in why this is so.
5)  The larger the pet (dog) the harder it is to find a driver to take it.
6)  Many drivers are allergic to cats.
7)  Others can only take cats.
8)  Some states are just plain difficult for most drivers as they do not contain the most heavily traveled interstates.9) Some areas of other states are also difficult due to the configuration of the state in relationship to major interstates.
10) There are times when a trucking company will assign a driver a load and that driver is enroute with it when the company will order that driver to exchange trailers with another driver.  Thus, the original driver now going another direction.  Drivers have NO control over this happening and, while it does not happen often, it can.  If your pet is enroute, the driver will keep you informed for these changes. 
      UNDERSTAND 

2PT11-4 OFFICE HOURS 

1)  The Operation Roger office is officially closed Saturday, Sunday, Holidays, AND also travel days to/at/fromtruck shows.  We are open generally from 9 a.m. - 5 p.m. CENTRAL time.
2)  However, DO submit your applications for transport during this time.  We will get to all applications as quicklyas possible, but be aware there may be a delay in processing your application.
3)  Currently, there is only one person in the office and that person must attend the truck shows.
4)  Due to lack of staffing, phone calls to the office at 682-622-1172 will only be returned IF a message is left. Since our phone is a Cell phone, and due to our physical location, we have found sometimes these calls do not show up on our phone for 2-3 days.  We apologize for this, but it happens.  Calls will be returned asap.
5)  If you do not receive a return call within about 24 hours, allowing for our office hours, follow up with an emailto support@operationroger.org. 
             UNDERSTAND 

2PT11-5 DRIVER HAS FINAL SAY
1)  Applicant/Shipper/Receiver understands the Operation Roger Volunteer Driver assigned to your pet has the finalsay on whether or not that pet is transported.
2)  If the driver determines the pet does not meet Operation Roger's or their own transport requirements, the pet maynot be picked up.
3)  If that should happen, Operation Roger will seek another driver better suited for your pet as quickly as possible. 

        UNDERSTAND 

2PT11-6 SHIPPER/RECEIVER REQUIREMENTS

1)    The Applicant/Shipper/Receiver understands that, following having contact with any Volunteer Operation RogerDriver, they are not to call them directly with any further transport needs.
2)      This puts the Driver in an awkward position of having to explain that ALL requests for transport MUST be doneon-line through the proper channels.  Please respect their privacy.
    UNDERSTAND 

2PT12-1 APPLICATION DONATION
1)  We only ask for a small minimum Non-Refundable but Tax-Deductible Donation to accompany this Application.  Operation Roger is a 501(c)(3) Non-Profit Charity.
2)  Many have asked why we do not require more. We prefer to keep it small to benefit the neediest of us.
3)  If you wish to add more than the minimum to the Donation, it would be greatly appreciated as these donations goto meeting our phone, internet, and office requirements first.  Without those services, we would not be here to help these pets get across country to better homes and lives.
4)  Donations from outside the United States: Consider the Exchange Rate and allow for this in order for OperationRoger to receive the basic Donation per pet.
5)  Donations through PayPal.  If donor does not have a PayPal account, no problem.

     A page will come up for entering debit/credit card information.
6)  Donation made by Someone Else: Name should also appear on the Application.
7)  Checks, Money Orders, etc.: Not accepted as the time lag required for these items to clear all banks wouldseverely limit how soon the pet could be put on the Active Pet Board and the transportation for it even worked. 8) PrePaid money cards can be used if accepted by PayPal.  Any refunds made at our discretion possibly may not be able to be credited back to them.
9)   Western Union: Make sure Operation Roger receives at least the minimum required Donation.  (PreapprovalRequired)
10)  For your convenience, a Donation for 2, 3 or more pets can be made at one time, the basic Donation times 2, 3,4, & 5 are shown below.
